Holdings, holding stack

Hello, i have two questions about holdings. 1. Are holding entries based on heading or track? 2. On holding stack, non radar environment, when the aircraft on holding are cleared for the approach? For example I‘m at the holding but the other aircraft is at the FAF, should the atc give me a clearance to start the approach or when the other aircraft is at the minimums. I would really appreciate if you could add references to books to answer the questions Happy landings.

    Holding entries are not based on heading or track. They are based on your magnetic direction from the fix. See AIM paragraph 5-3-8 (j)(3).

    AIM Paragraph 5-4-10 discusses timed approaches. They are not used unless a control tower is in operation. This would suggest coordination between the tower and approach controller, so that you clearance to execute the approach will not be given until the first airplane has landed.

    If you are pursuing an instrument rating, you really should become familiar with the AIM.
