I have two questions about this DP.
First, at the top right corner of the chart, there's a note "TOP ALTITUDE: 2000." What exactly does it mean? Does it simply indicate that pilots should stay below 2000 throughout the procedure until cleared for higher by ATC?
Second, the takeoff minimums section in the middle of the chart states "Rwys 11, 29: NA - ATC." Once again, what's the meaning of this note? Does it mean that the runway is not authorized for this DP for ATC purposes or that permission might be obtained after consulting ATC?
