How to contact approach control for class delta? (details in description)

I know I have to have to Give a cold call Ex. "Wilmington approach, Cessna 3030E" and then they come back and say "Cessna 3030E go ahead" at this point i know what I have to include, aircraft type (cessna 172,), altitude, location, intentions, and current atis. However, I just want to say this in more of a correct format. I don't know which goes where. ATC has never had a problem with me when I contact them, but I want to be a better communicator and say it in the right way.

    You may not enter a Class D airspace unless you first establish radio communications with the control tower. They must acknowledge you with your N number in order for radio communications to be established. Once you are in radio contact, then make your request. Example “Concord Tower, Bonanza 12A, 10 miles east at two thousand, landing with Bravo”.
